PMAD Is Not Always New:

When Old Trauma Resurfaces in Pregnancy

A 1.5-Hour Trauma-Informed Clinical Training for Mental Health Providers

This 90-minute clinical training examines the intersection of trauma history and perinatal mood and anxiety disorders (PMAD), with a specific focus on survivors of domestic violence, sexual assault, and forced migration.

Participants will explore how trauma reactivation presents during pregnancy and postpartum, how to differentiate hormonally driven mood symptoms from nervous system activation, and how to conduct culturally responsive, trauma-informed assessment in perinatal care settings.

This course moves beyond checklist screening and toward clinical precision. Providers will gain practical strategies for trauma-informed interviewing, safety planning, interdisciplinary collaboration, and case conceptualization when working with perinatal populations with complex trauma histories.

By the end of this training, participants will be able to:

  • Differentiate between hormonally driven perinatal mood symptoms and trauma reactivation in survivors of violence and displacement.

  • Identify how domestic violence, sexual assault, and immigration-related trauma influence perinatal mental health presentation.

  • Apply trauma-informed assessment strategies during pregnancy and postpartum clinical care.

  • Integrate culturally responsive safety planning and interdisciplinary collaboration into perinatal treatment planning.

Let's break it down

  • Module One
    Rethinking the Timeline of PMAD
    - Explore why PMAD does not always begin in the immediate postpartum period. This module reviews research on delayed onset, recurrence patterns, and extended vulnerability windows beyond the first year.

  • Module Two
    The Missed Populations
    - Examine groups frequently overlooked in screening and treatment, including refugee and immigrant mothers, Black and Brown women, military families, and individuals experiencing social isolation or economic stress.
  • Module Three
    Trauma, Migration, and Cultural Context
    - Understand how displacement, acculturation stress, historical trauma, and systemic barriers influence symptom presentation and help-seeking behaviors in perinatal populations.

  • Module Four
When PMAD Looks Like Something Else
    - Differentiate PMAD from adjustment disorders, grief, burnout, or relational distress. Learn how symptoms may be masked by high-functioning behaviors or cultural expectations.

  • Module Five
Screening Gaps & Clinical Red Flags
    - Identify why standard screening tools may miss symptoms. Review practical strategies for deeper assessment, culturally responsive questioning, and recognizing subtle warning signs.

  • Module Six
    Treatment Considerations & Ethical Practice
    - Review intervention strategies that integrate trauma-informed care, culturally responsive treatment, and interdisciplinary collaboration. Discuss ethical considerations when working with vulnerable perinatal populations.

  • Module Seven
    Systems-Level Advocacy & Prevention
    - Explore how providers can advocate for broader screening windows, policy awareness, and improved access to care for underserved perinatal communities.
